30 Cases of Bechterew¢¥s Disease


Abstract
30 cases of Bechterew¢¥s disease has been observed in the N.M.C. during the period of 20 months, 29 cases of male and 2 cases of female, most prominent in the young men between 10-30 years of age. No single case of below 10 and above 40 years of age in the onset of disease has been observed. No hereditary, neither traumatic history were found.
Common clinical symptoms were stiffness of spine, back pain and general malacia without any special neurological finding. Blood sedimentation rate was high in almost of the cases and it seems more higher in more advanced cases. No anemia, no abnormal blood chemistry were noted. 3 pulmoary tuberculosis patients, one heart disease and one gynecological case were noticed as complications.
Osteoporosis is observed in 20 cases and it seems not happened to the early cases before 3 years from the onset of this disease. Involvement of arthritic change of sacroiliac joints was found in 24 cases and the 6 cases were free from the affection. Without exceptionary apophysial joints of the lumbar spine were fused, 21 cases were involved in the apophysial joints of thoracic spine out of 28 cases, 5 cases in cervical spine out of 15 cases examination. Paravertebral ligament calcification were seen in 24¢¥ cases of lumbar spine, 10 cases in thoracic spine and 5 case in the cervical spine. Only 2 cases were indicated for deep x-ray therapy. 13 cases of arthritic change in hip and knee, only 2 cases in shoulder were observed. No single arthritic change of hand or feet was found.
Differential diagnosis between Marie-Striimpell¢¥s disease and Bechterew¢¥s disease is quite impossible. Exact etiology of this disease still, unknown, but it seems that Bechterew¢¥s disease is quite different entity from rheumatoid arthritis considering the sex distribution and also affecting joints. No single paying patient was involved by the disease contrary to the rheumatoid arthritis where many paying patients. have been involved. Sexual hormonal factor as well as low living standard may be ?_Aded to the etiology of rheurrra.-toid arthritis as the etiology of this disease.
Bechterew¢¥s disease ,has been inclined to stay away from the doctor¢¥s attention in Korea. Early exact diagnosis of this disease may cure or stop the progress of this disease by x-ray deep therapy. Bechterew¢¥s disease is not a rare disease at all in Korea.
